Late Appointments
When you book an appointment with Path 2 Healing a time has been set aside for you as per your request. We ask that you understand the importance of being on time for your appointment and respect our need to stay on schedule to ensure we can cater to other clients. In the case of late arrivals, it should be understood that only the time remaining for your scheduled appointment will be allotted unless additional time is available.
Path 2 Healing's policy for missed or late appointments is simple. If you do not show up for your scheduled appointment, and you have not provided notice at least 24 hours in advance, you will be required to pay the full cost of the treatment as booked.

Cancellations
In the event that you are unable to keep your scheduled appointment, please contact us by email or phone at least 24 hours prior to your appointment. If you do not reach us, please leave a message on our voice-mail. If we do not hear from you within this time frame, and you do not keep your appointment, the above policy will be applied.
General Administrative Policies
A patient health history form will be required prior to your first treatment. A renewal of this form will be necessary annually and after an absence of one year from your last treatment. We ask that you please arrive 15 minutes prior to the start of your first treatment to allow time to complete the necessary paperwork.
All information discussed and recorded will be completely confidential. Your written authorization is required for the release of any personal information pertaining to your file.
All patient information will be securely stored for 10 years following the last treatment. If the patient is under the age of 19, the information will be retained for 10 years following the patient's 19th birthday. Once the appropriate time has elapsed, the contents of the patient's file will be properly disposed of.
An "appointment" may include a patient intake, assessment, treatment and assignment of remedial exercises.
The patient may terminate treatment at any point during the appointment, at their discretion and without reason.
Payments may be made in either cheque or cash form and a receipt will be issued following treatment.
Cheques returned (NSF) will be subject to a service fee of $25.00.
Letters pertaining to legal or medical situations or summary of treatments will be for a charge of $25.
